Supports Coordinator - 176179

Piper companies is seeking a Supports Coordinator to support individuals with Intellectual Disabilities (ID) and autism spectrum disorder (ASD) throughout the greater Philadelphia area. This role will partner with individuals, families, and service providers to coordinate care, ensure health and safety needs are met, and advocate for access to services that promote independence and quality of life.

Responsibilities for the Supports Coordinator:

-         Coordinate services and manage a caseload of individuals with ID/ASD

-         Conduct regular visits to assess health, safety, and service needs

-         Collaborate with families, providers, and community resources

-         Monitor service delivery and address concerns or gaps in care

-         Maintain accurate documentation, reports, and case notes  

Qualifications for the Supports Coordinator:

-         Bachelor’s degree required; Human Services-related field preferred

-         Strong communication and relationship building skills

-         Experience with case management, social services, or healthcare preferred

-         Excellent organizational and documentation abilities

-         Valid driver’s license, reliable transportation, and auto insurance required

Compensation for the Supports Coordinator include:

-         Salary: $42,000-$46,000 depending on experience

-         Comprehensive benefits package including medical, dental, vision, and retirement plans
